Clinical presentation: A 30-year-old man was referred for evaluation of a thyroid nodule detected on screening.

Palpation: a firm nodule in the left lobe.

Functional state: euthyroidism (TSH 1.38 mIU/L).

Ultrasonography. The thyroid was echonormal. There were two hypoechogenic nodules in the left lobe, one in the middle and another one in the lower part. The former presented microcalcifications, while both did irregular, blurred and lobulated margins.

Cytology was performed from the nodule in the middle part of the left lobe and resulted in papillary carcinoma.

Histopathology disclosed papillary carcinoma.
