Nodular goiter

Clinical presentation: a 54-year-old patient was referred for an evaluation of a 'nodular goiter' detected on a sonographic screening.

Functional state: subclinical hypothyroidism (TSH 5.18 mIU/L, FT4 12.4 pM/L).

Ultrasonography: the thyroids contained multiple hypoechogenic areas. These do not fit a nodule because of their shape. See longitudinal sections and video.

A-TPO was below 10 IU/ml.

FNAC disclosed Hashimoto's thyroiditis.

Comment: it is worth to analyze the video and the longitudinal section of the left lobe. In contrast with a true nodule, these hypoechogenic areas are irregular in shape.
