Subacute granulomatous de Quervain's thyroiditis - Case 30.

Clinical data: a 56-year-old man was referred for the evaluation of neck discomfort lasting for several weeks. Body temperature was normal.

Palpation: both thyroids were tender. There was a nodule in the left lobe.

Functional state: subclinical hyperthyroidism with TSH 0.04 mIU/L, FT4 19.9 pM/L, FT3 6.71 pM/L, ESR 49 mm/H, CRP 8.95 mg/L.

Ultrasonography: the right thyroid was moderately hypoechogenic and contained even more hypoechogenic lesions with blurred borders. The vascularization was decreased. There was a hyperechogenic nodule in the moderately hypoechogenic left lobe.

Cytological diagnosis resulted in subacute, granulomatous thyroiditis from the hyperechogenic area, while from the nodule it was a benign hyperplastic nodule.
