Clinical data: a 41-year-old man was referred for an evaluation of a newly discovered nodule. He was treated for myocarditis in intensive care unit for several weeks. CT scan demonstrated a nodule in the right thyroid.

Palpation: no abnormality.

Functional state: euthyroidism (TSH-level 0.29 mIU/L, FT4 14.9 pM/L, FT3 3.18 pM/L).

Ultrasonography: there was a moderately hypoechogenic lesion in the ventromedial part of the right lobe. Above the thyroid in the middle of the neck a cystic lesion was found.

The patient told us that a relatively large neck cyst was evacuated 5 years earlier.

Cytology of the thyroid lesion: benign colloid goiter.

Comment: median (thyroglossal) and lateral neck (branchial) cysts are relatively rare after the first two decades. If a neck cyst will be refilled after evacuation, surgery is indicated because of the 1% risk of cancer in the wall of the cyst.
