Clinical data: a 69-year-old woman was referred for an evaluation of a newly discovered nodule in the right thyroid detected on carotid Doppler examination.

Palpation: no abnormality.

Functional state: euthyroidism (TSH-level 0.68 mIU/L).

Ultrasonography: there was a moderately hypoechogenic lesion dorsal to the right thyroid.

Cytology: benign, may correspond to parathyroid adenoma.

Further evaluation of the patient revealed elevated calcium-levels in the serum and in the urine and elevated parathormone-level (122 ng/mL). The patient was operated on.

Histopathology: adenoma according to the right lower parathyroid.
