Clinical data: a 58-year-old woman was referred for an evaluation because of 15 kg gain in weight.

Palpation: both thyroids were moderately firm, but no nodule was palpable.

Hormonal investigation: indicated subclinical hypothyroidism with TSH-level 9.01 mIU/L, FT4 10.0 pM/L.

Ultrasonography: revealed moderately hypoechogenic inhomogeneous thyroids without any circumscribed area.

Clinical diagnosis: autoimmune thyroiditis, subclinical hypothyroidism.
