Clinical data: a 58-year-old woman requested a second opinion because a "thyroid cancer with great probability" was diagnosed on "somato-magnetic investigation" performed by a natural therapeutist.
Palpation: a firm nodule in the right lobe.
Functional state: euthyroidism with TSH 4.67 mIU/L, FT4 13.6 pM/L.
Ultrasonography: there was a moderately hypoechogenic nodule with irregular borders and combined type 2 and type 3 vascular patterns in the lateral part of the right thyroid. There were two lymph nodes above the right thyroid.
Cytology resulted in benign colloid goiter.
We advised follow-up examination in three year, but the patient wished to be operated on.
Histopathology disclosed follicular adenoma.
Comment. This case illustrates the difficulty of the cytodiagnosis of a follicular tumor: although the presence of colloid and the predominance of monolayered sheets stand against a follicular tumor, do not exclude this possibility.
