Clinical presentation: A 32-year-old woman was referred for aspiration cytology. She has been treated for hypothyroidism for three years. Her endocrinologist performed ultrasound and faced with a concerning pattern.

Palpation: no abnormality.

Hormonal investigation: TSH 1.80 mIU/L, aTPO > 1300 U/mL, ahTg 547 U/mL.

The dimensions of the area in question in the left lobe were 8x8x11 mm, width, depth, length, respectively.
