Clinical data
First examination
Clinical presentation: A 43-yr-old woman requested evaluation. She has got replacement therapy (daily 37.5 microgram levothyroxine) for hypothyroidism for 5 years. Three weeks ago, her thyroid lobes became swollen and tender. Subfebrility has also developed. She stopped the replacement therapy. Usual analgesics only temporarily relieved her complaints, two courses of antibiotics suggested by her GP were ineffective.
Palpation: Both lobes were hard and painful.
Laboratory tests: TSH 0.09 mIU/l, FT4 23.6 pM/L, aTPO 94 U/mL, CRP 33.5 mg/L.
Second part of the video was recorded 4 years later
Clinical presentation: After two 6-week courses of steroid therapy the patient remained free of complaints. Here thyroid function - after a temporary hypothyroidism - restored and the patient remained euthyroid without any thyroid medication. She came to a routine follow-up check.
Palpation: no abnormality.
Laboratory tests: TSH 4.18 mIU/l, FT4 12.3 pM/L.
