Clinical presentation: A 58-year-old woman was referred for thyroid ultrasound exam. She has been treated for hypothyroidism for three years.

Palpation: Both lobes were firm on palpation.

Laboratory examination: TSH 0.77 mIU/L on daily 75 microgram levothyroxine.
