There is a moderately hypoechogenic, inhomogeneous lesion in the right lobe. The extralesional part of the lobe is moderately hypoechogenic corresponding to lymphocytic thyroiditis. The irregular shape of the lesion, the similarity of the echostructure of the lesion and the thyroid favour that this circumscribed area is a secondary or pseudolobule and not a nodule in pathological sense.

To summarize: it is more probably that the lesion is not a nodule on ultrasound. On the other hand, the presence of hyperechogenic granules and the disturbed, irregularly increased intranodular blood flow are properties which raise the possibility of papillary cancer. Therefore, cytological evaluation is required. Cytology disclosed Hashimoto's thyroiditis.
