Note the similarity between the two cases. The case of medullary cancer presents more while anaplastic cancer less pleomorphism than in a usual case. On cytological analysis, a clear distinction would be very hard. Although the presence of elongated cells favour the possibility of medullary cancer, the spindle-cell variant of anaplastic displays similar cells. The key is the clinical presentation. Anaplastic carcinoma is the most agressive human tumor evolving over weeks. Naturally, if medullary cancer is involved in differential diagnostics, serum calcitonin measurement is essential.
