Clinical presentation: A 44-yr-old woman requested a follow-up. First, she noticed a nodule 2 years ago when aspiration cytology resulted in benign lesion.

Palpation: a not firm nodule in the left lobe.

Laboratory test: TSH 1.96 mIU/L.

Ultrasonography. There were several discrete lesions in the left lobe. The largest was composed of multiple nodules next to each other, which made the appearance lobulated. On the other hand, there were several suspicious protrusions on the margin, therefore the possibility of lobulated/spiculated margins had to be considered.

Cytology resulted in benign lesion.

Comment. This is not an equivocal presentation. First, there is a mass composed of distinct nodules, which makes the appearance of the borders lobulated. Second, there are some spiculations, which are likely within the nodule and not at the borders.
