Clinical presentation: a 40-year-old woman was referred for an evaluation with complaints suggesting hypothyroidism and a hypoechogenic nodule detected on ultrasound. She was treated with daily 100 microgram levo-tiroxin for 7 years.

Functional state: euthyroidism with TSH 2.01 mIU/L, FT4 17.0 pM/L.

Ultrasonography: both lobes were minimally hypoechogenic. There were multiple small lesions of different echogenicity and they were divided by fibrous tissue. The patient had the so-called micronodular or pseudonodular form of Hashimoto's thyroiditis. These lesions have no oncological significance.

FNAC: from a hypoechogenic lesion in the right thyroid disclosed Hashimoto's thyroiditis.
