The presence of halo is doubtful on gray scale mode, while there is perinodular blood flow on Doppler mode. This pattern is suspicious for a follicular tumor.
Nevertheless, the presence of a complete capsule is only one of the necessary properties in the histopathological diagnosis of a follicular tumor.
This lesion proved to be hyperplastic nodule on histology.

There is halo sign on gray scale mode. It seems at first sight, that the nodule presents parinodular blood flow. By thorough analysis of Doppler images, it is evident that the extranodular parenchyma presents extremely increased vascularization while intranodular blood flow is practically absent. It is equivocal whether the perinodular circulation corresponds to a true perinodular blood flow or does not.
Final histopathology disclosed an encapsulated papillary carcinoma, i.e. the nodule was surrounded with a complete capsule.
