Clinical presentation: A 54-yr-old woman requested a second opinion. She has been treated for hypothyroidism for several years. Two months ago, during evaluation of an upper airways infection, nodular goiter was described. Then, aspiration cytology resulted in Bethesda 2, and Bethesda 4 (suspicion of oxyphilic variant of follicular tumor), from lesion in the right, and left lobe, respectively.

Palpation: no abnormality.

Functional state: TSH 1.85 mIU/L on daily 50 microgram levothyroxine.

The lesion in question is the largest one in the left lobe. The dimensions of the lesion in the right lobe were 7x5x9 mm, while those of the lesion in the left lobe were 4x4x5 mm, width, depth, length, respectively.
