Clinical data: a 44-year-old woman was referred for an evaluation of newly discovered hypothyroidism.
Palpation: no abnormality.
Functional state : hypothyroidism (TSH-level 18.8 mIU/L, FT4 7.03 pM/L).
Ultrasonography: the thyroid was hypoechogenic and was composed of multiple hypoechogenic and echonormal lesions. One hypoechogenic circumscribed area in the left lobe contained microcalcifications. This lesion was aspirated.
Cytological diagnosis: suspicion of papillary cancer.
Histopathology: papillary cancer and Hashimoto's thyroiditis.
Comment: the cytological pictures are remarkable. We found no inclusion. The cause for suspicion was based on the presence of grooves.
