Clinical data: a 53-year old hypothyroid woman was referred for an evaluation of a thyroid nodule that had been discovered 2 months earlier.

Palpation: a solitary, firm nodule in the left lobe.

Hormonal examination: indicated euthyroidism on daily 75 microgram levo-tiroxine therapy (TSH 2.08 mIU/L).

Ultrasonography revealed a hypoechogenic inhomogeneous thyroid with a less hypoechogenic nodule in the left lobe. The presence of a halo sign was equivocal, but perinodular blood flow was detected on color Doppler mode.

FNAC report: was follicular tumor, with less than the average risk for malignancy. Surgery was performed.

Histopathological diagnosis: was benign follicular adenoma and Hashimoto's thyroidits in the non-nodular part of the lobe.
