The presence of Hashimoto's thyroiditis is unequivocal. The question is whether the possibility of a thyroid tumor has to be raised on the irregular arrangement of cells and the presence of a solitary inclusion. The cellular atypia itself has no relevance in the case of Hashimoto's thyroiditis.

Ultrasonography of the right thyroid

 

The whole thyroid is hypoechogenic inhomogeneous and contains several discrete areas. There is a relatively more hyperechogenic lesion with irregular hyperechogenic patches and granules including microcalcification in the upper central area of the left lobe. The lesion presents type 3 vascular pattern.


We gave a common cytological-sonographic diagnosis of Hashimoto's thyroiditis and suspicion of papillary cancer.
