The presence of Hashimoto's thyroiditis is unequivocal. The question is
whether the possibility of a thyroid tumor has to be raised on the
irregular arrangement of cells and the presence of a solitary
inclusion. The cellular atypia itself has no relevance in the case of
Hashimoto's thyroiditis.

The whole thyroid is hypoechogenic inhomogeneous and contains several
discrete areas. There is a relatively more hyperechogenic lesion with
irregular hyperechogenic patches and granules including
microcalcification in the upper central area of the left lobe. The
lesion presents type 3 vascular pattern.

We gave a common cytological-sonographic diagnosis of Hashimoto's
thyroiditis and suspicion of papillary cancer.
