Clinical presentation: a 44-year-old man was referred for an evaluation of a newly discovered nodule.
Palpation: a hard nodule in the left lobe.
Functional state: euthyroidism (TSH-level 3.02 mIU/L).
Ultrasonography: the left lobe was composed of a large hypoechogenic nodular area with microcalcifications and intranodular blood flow.
Cytological diagnosis: papillary cancer.
Histopathology: papillary cancer with intrathyroidal metastasis.
