Clinical data: a 48-year-old woman with a goiter known for years and growing slowly. A solitary nodule was palpated within the enlarged right lobe.

Functional state: euthyroidism with TSH-level 2.17 mIU/L.

Ultrasonography: a solitary hypoechogenic nodule in the right lobe.

Cytological report: tumor with follicular structure. Suspicion of (follicular variant of) papillary cancer.

Histopathology: encapsulated papillary cancer, follicular variant.
